Mountainlands Community Health Center --
Patients Served
Insured and Uninsured Patients
Mountainlands serves patients who are fully covered by health insurance or dental
insurance, as well as patients covered by medicaid or Medicare. In addition, many of our
patients have no medical or dental insurance. Patients who have insurance must present
their medical insurance cards at the time of service. They will then pay either the
insurance co-payment or our sliding fee, whichever is less.
Homeless Patients
To qualify for free services as a homeless patient, you must meet strict criteria for
homelessness. We will determine who is eligible to receive homeless benefits at
Mountainlands.
Migrant Laborers and Undocumented Aliens
The fee schedule for uninsured migrants and undocumented aliens is the same as the
reduced fee schedule for all uninsured patients. The fees for these services are based
upon family size and income.
We have no funds to pay for services patients receive at other facilities. This
includes hospitals, laboratories, x-ray facilities, etc. The patient should discuss
their situation with the outside facility prior to receiving service to determine whether
special assistance is available.
We conduct necessary lab tests for our established patients if, and only if, the tests
are requested by one of our practitioners. We do not have a fully-equipped medical
laboratory, nor do we receive funding to cover tests for outside providers. However,
if authorized by the patient, we will be glad to share with other providers the results of
any lad tests performed at our clinic.
