Cost of Services

Initial visits and medically necessary procedures are always billed at full cost or at a reduced fee if the individual or family has prequalified for this dicount (see Eligibility).  All services require a copayment due at the time of service.  Under unusual circumstances, the Eligibility Coordinator can authorize a one-time-only delay of payment.  This delay is available only to established patients and those who have already qualified for reduced fees.

We will also perform minor elective surgeries including some birth control procedures.   The fees for these elective procedures are higher than for essential medical services and are based on percentages of the standard fee.  Payment for these procedures is due at the time of service and may not be deferred.

Funding Sources

We are not a government clinic.  About 20% of our funding comes from a Public Health Service grant from the Federal Government, and about 20% is donated by Intermountain Health Care.  The remaining 60% comes from insurance and patient payments.
